Description: Linn County, IA is presently looking of HOME HEALTH CARE AIDE (3/25/13) right now, this job will be placed in Iowa. For complete informations about this job opportunity please read the description below. Part-time position (20 hours/week) to provide care to agency clients in order to facilitate independent living. Works under the supervision of the Home Health Service Coordinator (Gener! al). Assists in implementing the case plan by performing specific tasks under the direction of the Coordinator. These tasks will be determined at the time of assessment and be incorporated in each individual case plan by the Coordinator. Recognizes and reports any change in physical, mental or social well being of the client to the Coordinator in a timely manner. Notes the clientâs nutrition, orientation/confusion, safety in the home, and physical abnormalities or changes in clientâs condition. Daily contact with Home Health Office to report on clients. Documents activities and observations for clientâs record after each visit. Documents appropriately and legibly. Turns in all paperwork on time as requested by the Coordinator. Attends training programs and in-service activities. Continues to upgrade and develop the skills required in the functions performed. Observes confidentiality in all aspects of client care. Performs all other related duties as assigned.
! Qualifications:
High school diploma or equivalent ! required. Current Certified Nursing Assistant (CNA) Certificate documenting satisfactory completion of 75 hour CNA course and Completion of State approved Homemaker-Home Health Aide Training (12 hours) required. Acceptable substitutions for the CNA Certificate include: completion of training or in the process of receiving training required for LPN or RN licensure; or possession of an Associateâs Degree or higher in social work, sociology or other health or human services and a minimum of six (6) months experience providing care in a home care or nursing home setting. Ability to work independently in the clientâs home or other appropriate site, under the supervision of the Coordinator. Emotional maturity and ability to use sound judgment in dealing with clients. Good physical health. Ability to function as part of a team of professionals. Ability to transfer clients from bed to chair, carry groceries from car to home, push vacuum cleaner and other physical requirements t! o care for clients. Good written and oral communication skills. Valid State of Iowa Driverâs License required. Personal transportation and auto insurance. Must pass criminal records check and child and dependent adult abuse checks. Must pass a County physical examination which includes a drug test after offer of employment. Regular work attendance required.
Shift:
Flexible hours between 8:00 a.m. â" 6:00 p.m . Monday â" Friday
