The VA Central Iowa Health Care System provides a full continuum of care between the Des Moines campus and our Community Based Outpatient Clinics (CBOCÂ's) and is one of the Medical Centers that make up the VA Midwest Health Care Network (VISN 23). Employee selected may be expected to perform duties at other duty sites within VA Central Iowa Health Care System based on the needs of the organization.
Note: The full performance level of this vacancy is GS-8. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-7 to GS-8. The Professional Standards Board (a peer-review group) will review the selected candidateÂ's education and experience and recommend the grade and step at which the candidate will be placed. Duties:
The staff therapist is responsible for providing basic and advance respiratory care services to adult and geriatric patients at VA Central IA Healthcare System. However, it is necessary for the therapist to be knowledgeable regarding the basic treatment of all age groups of patients. The therapist plans, implements and evaluates the effectiveness of respiratory care and must be able to operate a variety of equipment. The incumbent function includes t! he responsibility to act as charge/lead therapist in the absence of supervisory personnel and will have full responsibility to modify the workload to accomplish the assigned duties; reviews, collects and evaluates relevant clinical data; performs both basic and advanced therapeutic procedures for adult and geriatric patients; assist the physician in formulating an appropriate respiratory care plan based upon patient assessments such as lung auscultation, blood pressure measurements, hemodynamics and cardiac output. Preferably can perform polysomnography (PSG) studies. PSG will include patient setup, monitoring multiple physiological data throughout a PSG, troubleshooting PSG equipment, identify and treat sleep related breathing disorders. Have ability to recognize the need for medical intervention if patient safety is at risk. Ability to stage and score sleep studies. The registered therapist will be involved with patient care planning and/or patient care conferences; commu! nicates changes in the Respiratory care plan with other healthcare team! members; provides effective education to patients, family, students, and co-workers as appropriate; performs both fundamental and advance diagnostic procedures including pulse oximetry arterial puncture, blood gas analysis, pulmonary functions, etc; performs emergency (includes Out of OR Airway Management/intubations) and acute respiratory care procedures in accordance with the quality guidelines in the departmental policy and procedural manual; actively participates in departmental quality assurance and hospital-wide organizational performance improvement activities. Maintains clinical and professional competency based upon established standards of practice; and assemble, checks, and corrects malfunctions and performs quality controls on Respiratory Care equipment.
The RRT practices professionally, must be reliable, able to work independently, be self-motivated, is customer service oriented and believes in the VA mission. The RRT is responsible for performing job! duties in accordance with mission, vision, and values of the Central Iowa Healthcare System. Advanced Cardiac Life Support and Basic Life Support certifications are preferred.
QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
- U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Certificate as an RRT from the NBRC (National Board for Respiratory Care) and a registry number, or a certificate from another body which the NBRC recognizes as its credentialing equivalent. This includes registration based on either:
(2) Having fulfilled the requirements and passed the registry examination for a registered inhalation or respiratory therapist administered by the (1) National Board for Respiratory Therapy from July 1974 through 198! 2 or (2) American Registry of Inhalation Therapists from 1961 through J! une 1974..
- Proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Familiarity with Phillips V60 non-invasive ventilation.
Familiarity with Maquet Servo-i invasive ventilation
BCLS certification is required, ACLS preferred
Experience with an electronic patient record system
Experience with PSG studies, including portable home units
Out of OR airway management, including intubations

GRADE DETERMINATIONS:
GS-7. None beyond the basic requirements.
GS-8. Candidates must have all of the following:
(1) At least 1 year of successful experience related to respiratory therapy at the GS-7 level or equivalent. Qualifying experience at this level is work which includes duties such as: administering assisted and c! ontrolled ventilation to patients with tracheotomies and other complex medical problems requiring frequent adjustments in ventilator parameters; administering and monitoring advanced ventilator techniques such as positive end expiratory pressure and continuous positive airway pressure; developing plans for weaning patients from ventilators; assessing the respiratory status of patients using data acquired through physical observation and clinical analysis of blood gas data, chest x-rays, and electrocardiogram to determine the effectiveness of therapy being administered and to make recommendations to physicians regarding changes in treatment; performing the most difficult respiratory therapy procedures to maintain ventilation, including endotracheal intubation, tracheal lavage, and tracheotomy care; and providing in-service training to others in various specialized areas of respiratory care.
(2) Demonstrated knowledge and ability needed to perform complex respiratory! procedures with minimal supervision. Is able to participate with physic! ians and nurses and other staff in planning respiratory treatment, with great reliance on the individualÂ's knowledge of the equipment.
(3) Demonstrated knowledge and ability to provide emergency or critical respiratory care; service as a shift leader or supervisor or sole responsible respiratory therapist on a shift; and to plan and conduct training sessions with respiratory therapy students, hospital staff, patients, and family members.
