Plan, implement and maintain a bereavement program to meet the needs of families/caregivers, as well as regulatory requirements for the Medicare Conditi! ons of Participation.
Develop educational programs and materials for patients, families, staff, and the community on loss, grief, and coping with bereavement.
Plan, implement and facilitate bereavement group events.
Provide bereavement services to all hospice program families, contracted nursing families, and be available for community needs.
Complete psychosocial assessments, diagnostic assessments (DSM-IV) and develop a plan of care for patients and/or families experiencing emotional distress.
Qualifications/Experience
Masters degree in theology, social work, or Mental health.
Licensure in the state of Iowa as a social worker, marriage & family counselor, or pyschologist.
Minimum of two years experience working with patients/families/caregivers through death and dying issues.
Proficiency with computer applications (i.e. Excel, Word, Access)
Strong interpersonal skills.
