For detail informations about this occupation opportunity kindly see the descriptions. Case Management is a service provided to individuals with a disability who are eligible for Medicaid. Case management is an individually focused process through which clients are assisted in identifying, securing,! and sustaining resources necessary to live as independently as possible.The Case Manager will develop, implement and maintain consumer support and documentation. This includes processing requests for service by obtaining, reviewing and analyzing necessary information about the consumer, utilizing information from the consumer, referral sources and ohter relevant data. The Case Manager will complete the individual assessment and social history and documentation annually, and will develop the Comprehensive Consumer Service Plan with the interdisciplinary team. The Case Manager will coordinate the implementation and monitoring of the service plan by advocating for and assisting the consumer in receiving services which were identified in the planning process, will initiate funding, complete quarterly progress reports. The Case Manager will communicate regularly with the consumer and service providers regarding progress and will complete case notes to document communication.The! Case Manager will complete billing and time reports, such as ! monthly billing and daily documentation in a timely manner.This position is Monday -Friday 8:00 AM-4:30 PM. Education Bachelor's Degree in Human Services field from an accredited college.A License to practice as a registered nurse and at least three years of experience in the delivery of services to peope with disabilities may be substituted for education.Experience A minimum of one (1) year of related full-time paid employment with both Chronic mental illness and Intelectual Disability populations, provided this work experience is direct client supervision for people with disabilities.Skills/AbilitiesDesirable to have leadership or supervisory experience. Desirable to have computer experience.Must be able to plan, analyze, synthesize, and write reports; develop and implement Comprehensive Consumer Service Plan. Schedule, observe, record and assess functioning in the areas of work, home, and community; perform basic math, communicate effectively and work with a variety of p! eople in a professional manner. Must be able to demonstrate competency in confidential client matters and advocacy. Must be able to organize, prioritize and plan work to meet deadlines. Must be able to demonstrate professional responsibilities and learn and apply new concepts and ideas. Must have a valid drivers license, a good driving record and be insurable.
