As a member of the interdisciplinary team, responsible for the coordination of patient and family discharge needs which involves collaboration from various members of the health care team across the continuum of care.  Performs the Utilization Management process for patients and families with their requesting payers for the efficient delivery of quality patient care. The successful completion of these duties contributes to the enhanced quality of cost effective discharge from Blank Childrenâs Hospital. Â
Responsibilities
Clinical Excellence/Patient Care
- Monitors admission and continued stay of all patients regardless of level of care.
- Monitors avoidable days which are delays in treatment or timely patient discharge, patient care inefficiencies, and/or barriers to treatment and/or discharge.
- Performs admission and continuing stay reviews for third party payers.
- Collaborates with patientâs physician regarding discharge goals to develop discharge plans.
- Collaborates with health care professionals internally and regionally to promote a smooth transition to home.
- Maintains clinical competency using the nursing process to provide quality care.
- Coordinates appropriate medication and/or home care services /supplies.
- Arranges follow-up appointments, activities, and schedules therapies.
- Coordinates transfers to other healthcare facilities to ensure compliance with regulations, i.e. EMTALA.
- Meets with healthcare team to facilitate, coordinate, and evaluate patientâs discharge planning needs.
- Offers education about level of care criteria to members of the healthcare team.
- Assists in the evaluation of healthcare team as requested.
- Demonstrates ability to consistently meet the department workload and schedule. Â Â Â Â
- Utilizes a family-centered care approach (considering physical, psychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service area(s).
- Operates computer system to record and retrieve appropriate information.
- Prioritizes dissemination of information using available technology.
- Manages supplies i.e., order, obtain and store supplies.
- Efficiently and accurately maintains patient records. Â
Clinical Excellence and Patient Education
- Develops a discharge plan with collaboration of the family and the multidisciplinary health care team.
- Offers education about level of care criteria to members of the healthcare team.
·        Consults appropriate resources within the hospital to provide specialized education. Â

·        Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.  Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.  Takes appropriate action on concerns reported by department staff related to compliance.
·        This position may be assigned a specific mission to address in an emergency situation to assist in disaster response and recovery as part of the UPHDM Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained.

Qualifications
Education: Â Â Â Â
Graduate of accredited nursing program.
BSN must be obtained in 5 years
MSN preferred
Experience: Â Â Â Â
Minimum 2 years clinical pediatric experience working with multidisciplinary teas Â
License(s)/Certification(s): Â Â Â Â
Current licensure in good standing to practice as a Registered Nurse in Iowa.
Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire. Â
Valid driverâs license when driving any vehicle for work-related reasons. Â

Writes, reads, comprehends and speaks fluent English.
