JOB SUMMARY:

The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures and the Nurse Practice Act. The Wellness Director must exhibit genuine interest in residents and show patience and understanding while working with residents, families and visitors. This position oversees the day-to-day healthcare operations of the community support services and directly supervises the health care given, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident’s opportunity to remain in the assisted living environment. The Wellness Director oversees and promotes the residents’ health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident, resident’s family and MorningStar care team.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform the duties and responsibilities according to MorningStar’s Standards. The requirements listed below are representative of the knowledge, skills and abilities required.

License/Certification: Current Nursing License issued by appropriate state licensing agency and must have a First Aid certification as required

Education/Experience: Prefer a minimum of 2 years experience working with the elderly in an assisted living, skilled nursing or hospital environment. Basic typing skills along with a working knowledge of personal computing and word processing software

Continuing Education: As required by law and attend in-service educations within the community

Professional Memberships: Recommended and encouraged

Other: Must have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals, etc. Must have good communication skills and must be able to effectively communicate in English and understand and follow written and oral direction. Provide maximum resident-care services within limits defined by delegated tasks

STANDARDS: Perform duties in accordance with established methods and techniques and in conformance with recognized standards.

JOB KNOWLEDGE:

  • Knowledge of procedures and techniques involved in administering simple treatments and providing related resident-care service
  • Understanding of standard techniques used in providing personal services for resident and in caring for equipment and supplies
  • Good understanding of basis of universal precautions to avoid infection of residents and self or contamination of equipment and supplies
  • Be aware of the assessment process, and the development and execution of negotiated care plans.

DUTIES AND RESPONSIBILITIES:

Under the supervision of the Executive Director and the Regional Director of Resident Care:

  • Supervise the Assisted Living Coordinator and Reflections Coordinator.
  • Manages the staffing needs of the Assisted Living Community by interviewing, hiring, training, and supervision of MCMs.
  • Assists the Business office manager in the completion of new hire paper work, monitoring time sheets, and performing MCM evaluations per company policy
  • Manage the health care of all residents, including the dissemination of information to families, staff, physicians and third party providers. Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs
  • Provide maximum resident-care services within limits defined by delegated tasks
  • Perform duties in accordance with established methods and techniques and in conformance with recognized standards
  • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment, and maintain contact with resident’s family and discharge planner with the view to returning to the community
  • Complete pre-admission assessments for each resident and update every six months or as needed with change of condition
  • Initiate resident negotiated service plans according to the individualized needs of the resident as prescribed by physician and/or community policy. Provides resident and family instruction. Where applicable, seeks input from the resident and family in the development of the negotiated service plan
  • Performs ongoing assessments of residents’ physical and psycho-social needs and coordinates with other departments to assure quality, proactive care
  • Meet with each resident on a regular basis to provide health and wellness, check and answer any health-related questions
  • Review the resident’s medical report prior to move-in to ensure that MorningStar can provide for the resident’s individual needs and that regulatory requirements have been fulfilled
  • Order medications and oversees the resident’s medication schedule for residents receiving medication administration services
  • Assure that all medications prescribed are available by auditing the medication records and cart twice weekly and ensure proper documentation for medication assistance administration
  • Provide training, supervision, and monitoring of MCMs in the administration of medications, to include weekly auditing medication sheets, medication passes, shift change MAR audit and Narcotic counts. Performing MCM checklist prior to a MCM administering meds, and every 6 months
  • Coordinate additional health services the resident may need
  • On call duties for staffing & scheduling and works as MCM when other MCMs are not available. Provides coverage of the position in cases of emergency to ensure medication assistance is provided
  • Review changes in resident conditions with the Executive Director and resident’s physician
  • Conducts pre-admission screening of prospective new residents with the Assisted Living/Reflections Coordinators. Monitor that all required documentation is completed prior to or upon resident admission, including nursing assessments, negotiated service plans, and other assigned forms
  • Observe residents; records significant conditions and reactions, and notifies Executive Director and physician of resident’s condition and reaction to drugs, treatments, and significant incidents
  • As appropriate, explain procedures and treatments to residents to gain cooperation, understanding and relieve any apprehension
  • Facilitate continuity of care for those residents receiving home health services
  • Provide training and education to resident care managers on an ongoing basis at least twice a year. This includes classroom in-services, and situation specific training
  • Supervise the maintenance of residents’ charts, review documentation performed by the resident care managers
  • Organizes or assists the Executive Director in organizing, developing, and conducting service plans reviews as required with appropriate resident care team members and resident families.
  • Monthly reviews of the residents’ record and make appropriate interventions for weight loss.
  • Complete the Nutrition tracker monthly and send to VP of Health Care Services by the 20th of each month, and addresses it with interventions at the monthly QA/safety committee meeting
  • Monitors the skin assessments of the residents. Assesses wounds and obtains physician orders for treatments and interventions working with home health providers. Completes the wound tracker monthly with interventions and review at the monthly QA/Safety committee meeting
  • Assures ancillary medical services such as podiatrist, doctor visits, dental visits, psych visits, lab, x-ray, ambulance, etc. are scheduled and followed through
  • Assures that family members are aware of resident’s need for scheduled physicals, dental, or eye appointments
  • Responds to life saving situations based upon nursing standards, policies, procedures and protocol
  • Reviews incident reports for completion, documents in health care record, looks for interventions, and assures that the physician and family have been notified. Works with the Assisted Living/Reflections Coordinators to complete the addendum to the incident tracker for reduction of incidents. Reviews at the QA/Safety committee meeting monthly
  • Ensure all infection control guidelines are met
  • Maintain residents’ health care files. Document per MorningStar standards
  • Facilitate OSHA training and assist employees with OSHA compliance
  • Oversee and assist with training regarding resident’s health and wellness
  • Provides safety, comfort, and well-being of our residents
