Provide routine, personal healthcare, such as bathing, dressing, or grooming, to mentally ill, developmentally disabled, brain injured or autistic persons in the site home and commun! ity. Provide support and teach individuals, enable them to work on and meet their Personal Plan goals, assist them with completing activities of daily living with the intent of the individuals becoming more independent. Provides oversight of assigned employees in the site home and performs other duties as assigned.

Essential Functions Statement(s)
- Provide and maintain, as required by state regulations and company policies, records of individual care, condition, progress, or problems to report and discuss observations with supervisor and/or nurse.
- Understands, follows, and advocates for the individual's rights and ensures that the individual
- De-escalate c! risis situations using Mandt techniques and principles.
- Ensures individuals' health, safety, and welfare needs are met.
- Assist individuals in ADLs, personal hygiene, laundry, housekeeping, meal planning, shopping, coping skills and recreation/leisure skills.
- Plan, purchase, prepare, or serve meals to individuals, according to prescribed diets.
- Accompany individuals to doctors' offices and in the community, providing transportation, assistance, and companionship.
- Possess complete knowledge of each individual's program plan.
- Provide training to new and existing site staff on active treatment schedules and individual program plans for each individual.
- Ensure appropriate adaptive equipment is available and in good working order.
- Direct individuals and train staff to provide support to individuals in simple prescribed exercises or in the use of braces or artificial limbs.
- Entertain, converse with, o! r read aloud to individuals to keep them mentally healthy and alert.
- Assist Supervisor to coordinate staff schedules.
- Conduct department orientation with new team members.
- Communicate with supervisor and peers regarding individual needs, training needs, team member concerns, and input on individual programming effectiveness and appropriateness.
- Attend Individual Program Plan meetings for each individual in the site. Provides support in completion of individual goals and activities of daily living.
- Assist with house meetings monthly.
- Complete monthly house inspections and complete needed paperwork
- Gather and QA documentation for the site
- Report and coordinate relevant incidents with the on-call supervisor.
- Other duties as assigned by department or shift.
SKILLS & ABILITIES
Education: High School Graduate or General Education Degree (GED) preferred.
Experience: Six months to one year related experience
Certificates & Licenses
Within the first 90 days, attend and pass all required training for the position. Attend any other training events as instructed by supervisor or if mandatory by agency.
Other Requirements
Must be at least 18 years of age. Able to pass TB test, physical, and drug screen prior to hire and as required by regulation and/or policy. Possess a valid driver's license and current auto insurance. Satisfactory results on background checks with previous employers, criminal, and abuse registries.
