Provide clinical leadership and expertise within the palliative care and hospice service lines to improve patie! nt care throughout the hospital and outpatient settings, for patients with serious, complex illness and end-of-life care.2. Builds effective relationships with referring physicians to ensure excellent team follow-up regarding patient consults and to build an improved culture of collaboration among specialists, primary care physicians, nurses, and other staff to meet the needs of palliative care and hospice patients.3. Proactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resource use.4.
Ensures compliance with legal, regulatory, and clinical policies and procedures working closely with the Director of Palliative Care and Hospice.Job Summary: â¢Participation in the interdisciplinary palliative care and hospice services teams and provision of direct patient care and appropriate related services to the patient and family. â¢Leadership in clinical oversight of Hospice Unit.â¢Leade! rship and clinical oversight of Ambulatory Palliative Care Cli! nic.â¢Participation and clinical leadership in the development and review of appropriate clinical protocols and processes that support appropriate referral and consistent care.â¢Leadership in the identification of education needs of the physician staff and development of effective outreach strategies.â¢Outreach to community agencies and health care settings (such as nursing homes) as appropriate.â¢Support and back-up to the medical director as appropriate.â¢Participate in 24 hour call structure.â¢Responsibility for working closely with staff physicians to ensure that the palliative care and hospice service is well integrated with existing capabilities and educational and quality initiatives.Hospice Care Job Responsibilities: â¢Participate actively in the screening and admission process, making â pre-hospiceâ and face to face recertification visits as necessary.â¢Offer primary patient management services to interested providers.â¢Make appropriate follow-up visits! on hospice unit.â¢Make home visits when appropriate.â¢Review current cases for efficiency of plan of care.â¢Be available to staff as a resource and for educational opportunities.â¢Improve community awareness and presence.â¢Weekly participation in interdisciplinary team meetings to review active cases in both hospice unit and community program.â¢Be available to hospice staff as a resource for pain and symptom management questions.â¢Full and appropriate documentation of direct patient care services to support regulatory requirements and professional billing for services. Hospice Care Job Responsibilities (continued): â¢Ad hoc availability to support other team members with clinical advice and direct assistance with physician communication needs.Palliative Care Job Responsibilities: â¢Participate in each new consultation as appropriate.â¢Make appropriate follow-up visits on the daily census.â¢Offer co-management services to primary providers as appropriateâ¢Full a! nd appropriate documentation of direct patient care services to support! successful professional billing for services.â¢Be available to hospital staff as a resource for pain and symptom management questions.â¢Ad hoc availability to support other team members with clinical advice and direct assistance with physician communication needs.Job Qualifications: â¢Current medical license in state of practice and in good standing with medical board.â¢Credentialed for full scope of practice at the hospital.â¢Appropriate board certification in a related specialty.â¢Experience with ethics consultations preferred.â¢Experience with quality improvement methodology preferred.
