The individual in this role is accountable for planning, supervising and coordinating all patient care activities and daily clinic operations in or! der to maintain a safe, efficient, financially responsible environment to provide the highest level of quality patient care. This individual manages hiring, staffing, disciplinary action, development and employment decisions related to clinic staff. The Clinical Coordinator executes and monitors the daily clinical, labor and supply utilization, and regulatory functions of the programs consistent with the State, Federal and Company guidelines. This individual provides oversight and leadership to the patient care staff and ensures compliance to Company policies and procedures.
PRIMARY DUTIES AND RESPONSIBILITIES: ï·
Ensure patient care activities in accordance with the Companyâs policies and procedures.
Execute protocols and quality initiatives to improve clinical and patient outcomes.
Provide clinical management, effective communication, and leadership in the execution and direction of quality services to patients, staff and medi! cal staff. Develop and maintain effective communication and wo! rking relationships with staff, co-workers, physicians, and patients.
Manage labor and supply utilization.
Lead, coordinate, supervise, monitor, delegate and document all aspects of patient care activities.
Collaborate with all disciplines, utilizing internal and external resources to provide optimum patient care.
Manage and monitor the delivery of services to patients ensuring compliance in the performance of all aspects of patient care in accordance with Company policy and procedures, State and Federal guidelines.
Oversee and direct all patient care staff to ensure adequate visual monitoring of patients by nursing and patient care staff during treatment. Ensure delivery of safe and efficient quality care.
Manage all home patient care staff and patients to ensure safe and efficient quality care.
Manage patient and staff schedule to maintain optimum staff to patient ratios in accordance with clinical census, ope! rational goals and regulations set forth by state and federal agencies.
Support, understand and implement the philosophy, mission and policies of the center and Company; implement and promote Company quality outcome initiatives and Company culture.
Demonstrate competent knowledge of State and Federal ESRD regulations and center clinical, administrative, and financial and quality policy and procedures.
Manage and ensure appropriate documentation of nursing process, Assessments and Plan of Care, admission and monthly progress notes, problem lists and medication updates. Ensure patientâs medical records are maintained in accordance with Company policy and regulatory agencies.
Manage and direct all clinic patient care staff, licensed and unlicensed, (e.g. hiring, termination, evaluations, disciplinary process and reporting) within Company guidelines. Provide clinical leadership, training/ delegation of training of new employees and ongoing ! education and training of all clinical staff.
Manage implement! ation of quality assessment and improvement activities making recommendations based upon clinical and patient outcomes.
Schedule center audits including CQI, medical records, fire & safety and emergency preparedness. Delegate to appropriate personnel.
SECONDARY DUTIES AND RESPONSIBILITIES:
Serve as Supervisor of Medical Records.
Active role in RV Difference and orienting new employees to RV Difference culture and programs.
Oversee/serve as member of CQI Committee, Center Governing body and oversee/serve as Anemia Manager in conjunction with Medical Director.
Implement patient and staff satisfaction surveys and report results to CQI committee.
Manage reporting of Network activities and complete reporting in a timely manner, which includes but is not limited to, payroll, census, Epogen log, and quality reporting.
Maintain, monitor and complete reports in FDSO8, QCS and QMS data bases to ensure complete,! timely, and accurate information. Review and approve clinic invoices; perform required month-end reporting.
Review treatment log, medication and treatment audit and medication discrepancy reports to ensure accuracy.
Must have and maintain current CPR or ACLS certification.
Perform other duties as assigned.
QUALIFICATIONS:

Must have current and valid State professional nursing licensure
Baccalaureate degree preferred (nursing, business or related field)
Minimum of 2 years of ESRD experience.
Certification in specialty required within one year of acceptance of position.
! Prior experience managing or supervising direct reports.
Must ! have demonstrated record of and commitment to safety; and dedication to excellence.
Must possess outstanding communication and interpersonal skills. Must be able to communicate professionally and effectively with other staff members, patients and physicians.
Able to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
Basic knowledge of computers, Word Processing software and database.

The employees may be exposed to fumes or airborne particles and toxic or caustic chemicals. The noise level in the work environment is usually moderate.
The employee must frequently lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, color differentiation, distance vision, peripheral vision, depth perception and ability to adjust focus. While performing the duties of this job, the employee is required to stand; walk and talk, hear and smell. The employee is frequently required to use hands to finger, handle, or feel and reach with hands and arms. He/she is occasionally required to sit; stoop, kneel, bend, crouch, or crawl.
