The registered nurse provides nursing services to hospice patients and their families.
Essential Functions:
Completes the initial patie! nt evaluation, coordinates the implementation of the plan of care and initiates the plan of treatment
Administers prescribed medication as indicated by patient assessment and physician orders.
Prepares written instruction for and effectively supervises the hospice aide and/or homemaker.
Communicates with staff and appropriate community resources to coordinate services for the patient and family.
Participates in education for patient/family and contract facilities/services.
Prepares clinical and progress notes. Maintains and accepts responsibility for clinical records. Assures confidentiality and safety of clinical records.
Responds to emergency needs of the patient.
Participates in quality improvement planning and process.
Participates in interdisciplinary group (IDG) meetings and implements nursing care plan accordingly.
Participates in education programs, in-service programs and volunteer training. Develops and maintains effective worki! ng relationships with team members and community partners.
Complies with productivity requirements.
Supports and promotes a culture of compliance and ethical business conduct.
Complies with and cooperates fully in all components of the organization's corporate compliance program.
Acts in accordance with and adheres to all applicable regulatory and legal guidelines.
Home-based:
Obtains informed consent for care and initiates admission process.
Counsels the client and family to meet nursing and related needs. Helps them accept responsibility for care.
Assists patients with activities of daily living and household responsibilities.
Coordinates/assigns volunteers.
Participates in the training/education of staff, volunteers and community agencies-medical professionals as requested.
Initiates appropriate palliative, preventive and rehabilitative procedures.
Evaluates referrals for services in the area.
Coordinates services provided by the home-based team.
Coordinates care a! nd acts as the point of contact for communication with facility-based patients.
Accepts 'on-call' duty as assigned.
Education/Experience:
Registered nurse.
At least 2 years recent clinical experience in medical/surgical, oncology or public health nursing preferred.
Knowledge/Skills/Abilities:
Knowledge of care of the terminally ill and universal health care precautions.
Skill in the management of pain and symptom control based on the philosophy and standard of palliative care.
Demonstrates a tactful, supportive and sensitive approach to patient/family needs.
Ability to plan, organize and utilize patient/family teaching principles with regard to health care needs.
Ability to exercise independent judgment, plan and organize.
Ability to communicate effectively orally and in writing.
Ability to access patient homes.
Supports and promotes a culture of compliance and ethical business conduct.
Complies! with and cooperates fully in all components of the organization's corp! orate compliance program.
Acts in accordance with and adheres to all applicable regulatory and legal guidelines.
Strong keyboarding and computer skills with the ability to learn work-related software.
Other Requirements/Certifications:
Current Iowa registered nurse license.
Current Iowa driver's license and auto liability insurance.
Compliance with continuing education requirements.
Current CPR certification.
CHPN (Certified Hospice and Palliative Nurse) to be obtained within 24-36 months of employment if in full-time or part-time status.
T.B. test every 4 years. Chest x-ray or screening as determined by employee health.
Physical Demands:
Exerts 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly.
Occasional bending, stooping, squatting, kneeling, fingering and gripping and reaching activities.
Ability t! o provide lifting, turning and ambulatory assistance to patients.
