At the Highest level, Catholic Hospice Employees must (a) be committed to Hospice philosophy and mission and (b) provide outstanding customer service, aligned with the Customer! Service Guidelines, to both internal and external customers. Additionally, be able to promote an awareness and understanding of positive ethical and moral principles consistent with the mission, vision, and values of Catholic Hospice and those required by law. Under physician's orders, The Registered Nurse is responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the terminally ill patient/family unit. The RN utilizes assessment, intervention and teaching skills in caring for the patient/family unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Commitment to Hospice philosophy and mission.

Commitment and Implementation of Catholic Hospice's "Customer Service Guidelines."

Ability to interact with the public and other employees in a positive manner and promote a cooperative working environment.

Coordinate several projects at the same time with competing timelines.

Demonstrate adherence to Catholic Hospice, Inc. polices and procedures and pertinent State and Federal Rules and Regulations.

Able to identify and provide ongoing assessment of the physical, psychological, social and spiritual needs of the patient and family and act accordingly or initiate appropriate intervention or referral.

Demonstrates knowledge of hospice levels of care, Medicare and Medicaid Hospice Benefits by providing appropriate documentation when the need arises.

Able to provide on-going assessments of physical, psychological, social and spiritual needs of the patient and family and initiates appropriate intervention or referral.

Actively participate in the care planning process, updating care plans according to the needs of the patient.

Ability to communicate effectively orally and in writing.

Able to provide education to a diverse group of caregivers audibly a! nd in writing, on how to properly care for patient/family care.

Participates in the team responsibility of providing patient care twenty-four hours a day, seven days a week.

Consistently able to work calmly and maintain effectiveness in situations of high stress.

Maintaining the ability to extend hospitality and patience while assisting patients, families, physicians and peers under stressful circumstances.

Computer literacy required with proficiency in word processing and general computer applications.

Attend home deaths as required and communicating with physician, funeral home, and team.

Provide accurate and appropriate documentation and submit daily.

Ability to lift and move patients according to accepted nursing standards of body mechanics.

Ability to travel to any location throughout South Florida as needed and expand territory as business needs dictate.

RESPONSIBILITIES:
Asses! s physical, psychological, social and spiritual needs of the patient an! d family within 24 hours of admission.

Completes accurate and appropriate documentation of patient/family services within 24 hours of visit and maintains accuracy of electronic medical record including accuracy of medication profile.

Coordinates care to the patient and family within the Interdisciplinary team and availability of all resources.

Collaborates with Nursing Home staff by initiating the written plan of care, maintaining professional management of hospice care and attending care plan meetings

Actively participates in quality assurance and performance improvement projects as directed by Team

Serves as case manager and advocate to the patient and family while collaborating with attending physicians, insurance case managers, skilled nursing facility staff, assisted living facility staff as well as all health related practitioners.

Administers medications and treatments prescribed by the attending physician and/or ! hospice physician.

Evaluates services provided by the Hospice Aide every two weeks with periodic on-site supervisory visits.

Attends and participates in Interdisciplinary Team Meetings as required.

Represents Catholic Hospice at community nursing functions.

Participates in hospice activities such as Memorial Services, Community Education, etc., as requested by the Team Manager.

Provide orientation on a regular basis to new employees.

Ensure effective communication with Interdisciplinary Teams.
Participate with on-call rotations.

Performs other duties as assigned by supervisor.

LANGUAGE SKILLS:
Ability to read, analyze, and interpret common information, reports and medical documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the Healthcare industry. Ability to effectively present information to management, public groups, referra! l sources or patients and caregivers. Ability to speak and write Spanis! h is strongly preferred.

MINIMUM QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Current Florida License as a Registered Nurse.

One-year clinical nursing experience required. Hospice Nursing experience preferred.

Excellent oral and written communications skills in English. Bilingual (English / Spanish) strongly preferred

Proficiency in word processing and general computer applications, including, but not limited to Microsoft Office. Experience in Suncoast or Medicare / Medicaid systems a strong preference

Must be 18 years or older

Pass the drug test

Ability and willing! ness to work a flexible Full Time schedule

Must be able to use! facsimile, write and transcribe information accurately

Must be able to provide own transportation to various locations in service areas as required by duties
