:

Nursing leadership position which coordinates patient care, management and education with a multi-disciplinary health team.

Provides and manages patient care activities to patients of diverse ages as defined: birth to death.

ESSENTIAL FUNCTIONS

:

·

Provides leadership as a role model in developing and implementing the team concept of patient care delivery.

·

Works independently with limited direct s! upervision in accordance with the Iowa Nurse Practice Act,

policies and patient care standards and professional judgment.

·

Evaluates with Quality Management/Utilization Review and Social Services appropriateness of resource utilization based on established admission, discharge and transfer criteria, initiating appropriate intervention and collaboration to bring about successful result.

·

Demonstrates leadership skills in quality improvement activities including performance improvement processes to facilitate clinical practice changes.

·

Provides input toward performance reviews on an on-going basis as well as annually.

May follow-up with patient at home or across the continuum.

·

Maintains clinical competencies to serve as mentor to staff and as a clinical resource to patient care personnel.

·

Collaborates with Unit Director for a plan for achievement and maintenance of skill! s/ competencies of team personnel.

CARE COORDINATION

·

Develops and revises patient care standards, pathways, policies and standard orders for select patient populations in conjunction with other care providers.

·

Provides consultation upon request with area of clinical expertise to nursing staff and other health professionals throughout the healthcare system.

·

Coordinates and conducts patient care rounds and family conferences.

·

Assist staff to develop patient/family education and discharge plan with the focus on keeping them updated on tests, processes and expectations as well as the longer term needs for post acute care.

·

Integrates episodic patient care with other services, programs across the continuum.

·

Supports research by assisting staff as needed in collecting patient specific data for approved projects and educating staff about interpretin! g research findings and educates and assists staff to apply research to! practice. Collaborates with the appropriate resources to identify and carry out specific research projects to improve patient care.

·

Consults with patient, family, physicians and other caregivers in dealing with end of life issues in order to bring about understanding and consensus for care plan.

·

Consults with Ethics Committee as appropriate.

·

Develops and presents educational sessions to enhance staff development.

EDUCATION

·

Provides orientation/education for staff, students and other hospitals and community organizations, as requested.

·

Provides patient/family or caregiver education and counseling in recognition and solution of physical, emotional and environmental health.

·

Maintains knowledge of equipment set-up, maintenance and use (i.e., IV pumps, monitoring equipment, drainage devices).

·

Acts as a mentor and coach to ne! w staff.

·

Collaborates with affiliated educational program staff to coordinate and facilitate student experience.

·

Writes competency and ensures staff competence.

MANAGEMENT

·

Manages health care members (i.e., registered nurses, licensed practical nurses, PCTs, SAs, NCS, etc.) in the delivery of patient care.

·

Adjusts staffing and prioritizes workload to meet patient/unit needs.

·

Facilitates efficient operation of unit activities.

·

Maintains knowledge of documentation requirements needed for employees’ files.

·

Participation in interviewing â€" decision making for hiring.

·

Participates in and gives input to disciplinary process.

·

Assumes Unit Director responsibilities during vacation and leaves as assigned.

MARGINAL FUNCTIONS

:

·

Delivers care in sele! cted situations in order to achieve goals for mentoring, demonstrating ! and establishing relationships with patients and/or staff members.

.

Qualifications

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

:

·

Bachelor degree in nursing required, or actively in pursuit of BSN degree (accepted into a BSN Program and/or currently enrolled). Expected to take at least six credits per year at Unit Director’s discretion for exception.

Alternative educational preparation may be considered by the Unit Director to meet the individual needs of the unit.

·

Three years of R.N. experience

·

Licensed by the State of

Iowa

as an R.N.

·

Proof of completion of Mandatory Reporter â€" Child and/or Dependent Adult Abuse training within three (3) months of hire.

·

BLS certified by the end of the orientation period (approximately six (6) weeks).
